Licensed Master Social Worker 2-Res School Transition Coord

POSITION DESCRIPTION:
Under the supervision of the Developmental Disabilities Program Specialist 2, the Residential School Transition Coordinator (RSTC) plays an integral role in facilitating planning for youth aging out of residential schools. The RSTC is responsible for coordinating the transition to adult OPWDD services for students aging out of residential school settings. The RSTC will be part of team that is expected to assist with this planning for students from across the 18 counties within the Hudson Valley, Taconic, and Capital Districts of Region 3. Across the 18 counties within Region 3, approximately 276 students will be aging out from residential schools in 2022-2024. This number is expected to grow as additional students are found to be eligible for OPWDD services. POSITION
Responsibilities:
Duties include but are not limited to:
o Will have continuous interface with residential schools, families/guardians, the individual, Central Office, and provider agencies around the supports needed for transition from the residential school. o Assists with person centered planning and ensuring connections to residential or in-home supports as well as ensuring identification of day services. o Responsible for monitoring and upkeep of the residential age out data base. o Participate in quarterly meetings for individual's residing in residential schools to ensure awareness of current needs. o Visit individuals at their residential schools to be familiar with theirs needs related to planning. o May be needed to participate in Fair Hearings. o Interface with the Certified Housing Team o Involved in efforts to identify providers to plan and develop services including but not limited to, presentations at the Residential Opportunities Committee and involvement in the Letter of Interest (LOI) process. o Ensure the required Preliminary Adult Service Plans are submitted timely and reviewed to ensure they meet the individual's needs. o Assist with completing Developmental Disabilities Profile 2 (DDP2) Assessments o Assist with reviewing Higher Needs Funding applications o Respond to requests for information from Leadership within time frames given. o Complete other duties requested by DDPS2, DDPS4 and Deputy Director.

Salary Range:
From $67897 to $87653 Annually

$67,897 - $86,140 Capital DDRO $69,410 - $87,653 Taconic DDRO Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; OR Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying. Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying. Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.Estimated Salary: $20 to $28 per hour based on qualifications.
